SPA ETIQUETTE.

Day Spa Diva does NOT tolerate any sexual behavior or comments. If any sexual behaviors or
comments are made during session, the session will terminate IMMEDIATELY and you will be
responsible for full payment.

Day Spa Diva arrives at your location on time as scheduled. It is expected that you and your guests will
be on time as well. For every twenty minutes that Day Spa Diva has to wait for you or guests to arrive so
that treatments can begin there will be a $15 fee and time will be taken off of your scheduled treatment.

All cancellations MUST be made within 36 Hours or there may be a cancellation fee of 50% of your
scheduled spa service.

A credit card is required to hold the date of your party. All Lil' Diva parties and Teeny Boppin' Parties
require a $100 deposit to cover the cost of supplies. The deposit will be credited towards your party.

If you enjoy your spa treatment it is standard to give your massage therapist or spa technician a gratuity of
15% to 20%.

A contract must be signed prior to all spa parties and events. To view the contract please click here

You must notify Day Spa Diva five days in advance of all massages and spa services you and your
guests will be receiving. This is needed so that your massage therapist/spa technician brings all
necessary supplies and equipment. If your guests are a no-show you are still responsible for full
payment. This policy is in place because Day Spa Diva plans ahead and hires independent contractors
to give the treatments and specifies the amount that the contractors will be paid. When independent
contractors come to your location they are expecting to be paid by Day Spa Diva.

There must be a space large enough for a massage table to be set up and for your therapist to maneuver
around. A private space is preferred so that guests may enjoy their treatments in a peaceful, tranquil
environment.

A health form will be required for each guest to fill out and sign prior to any treatment. This is to ensure that
there are no contraindications to any of our spa treatments. Attached to the health form will be a disclosure
form for you to review and sign.

A minimum of a 90 minute appointment is required to book a single appointment with Day Spa Diva.

Your massage therapist/spa technician will arrive between 30 to 45 minutes prior to your scheduled
appointment. This is required so that set up time is not taken away from your massage or spa treatment.

You must notify Day Spa Diva in advance if you are pregnant.
